The Child and Young Person’s Advance Care Plan Collaborative has produced a care plan. The content accurately reflects the NICE guideline on end of life care for infants, children and young people with life-limiting conditions. It also supports statement 1 in the NICE quality standard on end of life care for infants, children and young people.
