Public health stakeholder registration form
Please read the accompanying notes before completing this registration form, including how NICE defines stakeholders for the public health programme.
Individuals are unable to register as stakeholders for public health guidance. If you are an individual or an organisation that does not fit our definition of stakeholder, you can still contribute to the development of a clinical guideline or quality standard via an appropriate national, professional or patient organisation.
Completing the form
This is the form for registering interest in the public health programmes. Please name one person from your organisation as the contact. Indicate which topics you are interested in by ticking the box, and give a very brief description of why your organisation should be considered a stakeholder.
Sending the form
Stakeholders will be accepted at any time during the development process and can participate in all consultation stages. Please consult our website for detailed information on the progress of development and the consultation stages for each topic.
Your completed form will be submitted to the public health team. Registration is automatic, and we will not contact you unless your organisation does not fit our definition of stakeholder.
Once you have become a registered stakeholder we will inform you about each new relevant public health topic. If you move organisations, are no longer the NICE contact for your organisation or would like to be removed from our database for any other reason, please email phguidance@nice.org.uk

For practical reasons, only national organisations can register as stakeholders. Local patient, service user, carer, community and professional groups cannot register as stakeholders unless there is no national organisation that represents the group's specific interests. Nor can individuals register as stakeholders. However, individuals and local organisations are encouraged to participate through an appropriate registered stakeholder. The registered stakeholders for each guidance topic are shown on the guidance summary page.

The information you submit on this form will be retained and used by NICE and its advisers for the purpose of developing its guidance. The name of your organisation will appear on the Institute's website with all personal data removed in accordance with NICE policies.
