Rationale
The purpose of this indicator is to encourage practices to record the name and contact details of carers of people with dementia with the aim of improving communication between those involved in a person's care, including within the practice and with other teams (for example, out of hours services).

Specification
Numerator: The number of patients in the denominator with the contact details of a named carer on their record.
Denominator: The number of patients with dementia.
Calculation: Numerator divided by the denominator, multiplied by 100.
Exclusions: Patients aged 18 years and under. This indicator does not relate to recording contact details of professional carers.
Minimum population: The indicator would be appropriate to assess performance at individual general practice level.
