Minor changes publication

September 2024: The rationale section was updated. Information about the minimum population was replaced by that of the expected population size. The rationale section was amended to include current QOF data.

May 2023: We amended the specification (definition and statement on personalised care adjustments and exception reporting were added, and the information about exclusions was amended). The source guidance section was amended to reflect an update to the source guideline.

October 2020: We added a specification and information about the minimum population. We removed the introduction section, recommendation and quality standard statement wording.
